Step 1: Download your form fill it out. (Don't forget to have your coach go through it and have him/her sign the form) settle your payment.
Step 2: Scan and email a clear copy of the form and deposit slip to (Photo taken forms are not accepted) our team will then email back once verified.
PAYMENTS: if unsure with your total amount to pay, you may request for a billing statement from our team, bank details will be stated in the file. Otherwise, you may directly settle your payment and email us the deposit slip. Bank details are available in the registration from.
Note: The following will not be accepted by the team:
-incomplete registration form
-printed copies given to the rink
-unclear photo of the forms
-social media sent registration form
No confirmation email response from the team means your form has not been received. Kindly resend the form.
